Keep every field one to three sentences; a log nobody fills in is worth nothing.

- **Question:** What exactly was being decided? Phrase it as the question you argued about.
- **Decision:** What was decided. One sentence, no hedging.
- **Date:** YYYY-MM-DD
- **People:** Who was involved, and who owns the outcome. Note anyone who disagreed.
- **Reasoning:** Why this option won. Include the strongest argument against it, so future readers know it was considered.
- **Options considered:** The alternatives that were on the table, including "do nothing".
- **Revisit:** When (or under what conditions) this should be reviewed, if ever.

Tips that keep a log alive:

1. Log the decision the same day it is made. A week later the reasoning is already fiction.
2. Record dissent honestly. "Priya opposed, preferring the phased rollout" makes the log trustworthy.
3. Link the log entry wherever the decision came up (the Slack thread, the ticket, the doc).
4. If entries keep not getting written, the problem is the re-typing step, not your discipline.
